Services Coordination - Intake
Once EDN receives a referral a services coordinator is assigned and is required to do the following:
- Document the date of referral and gather general demographic information about the child and family.
- Contact the family within seven calendar days of the receipt of the referral to set up a face to face meeting at a time and place mutually agreed upon.
- Complete the face to face meeting within seven calendar days of the initial contact, unless the family requests a delay.
- Provide written notice and obtain written consent for the child’s initial screening (if applicable) and Initial Multidisciplinary Evaluation and Child Assessment.
- Continue to work with the family, according to their needs and wishes until a decision is made as to the eligibility of the child.
- Notify the referral source of the referral outcome, with the permission of the family.
During the face to face meeting the services coordinator shall:
- Share information about the child and the system of Early Development Network services
- Start to develop rapport with the family
- Determine if the family is interested in proceeding with the process
- Begin to identify with families their daily routines, activities, and supports
- Gather information to address the family's concerns and questions about their child's health and development
- Assist families in beginning to identify their resources, priorities and concerns
- Offer referrals to community resources to meet immediate needs
