Specific Outcomes

General Outcomes

Family Outcomes:

  • Parent will have full-time employment outside of home
  • Parents will have had at least two breaks from full-time responsibilities of care-taking for children
  • Kids will be going to bed at a reasonable time
  • Family will have primary responsibility for coordinating Sophia's care
  • Mother will be aware of where child is develomentally in speech and language and know what to expect him to do in the future
  • Respite care will be arranged to allow parents to maintain employment and school enrollment & to provide a break from responsibilities of care-giving
  • Services Coordinator will work with IFSP Team to access resources that will benefit the family
  • To assist and empower parents
  • Work together to provide Family-Centered Services for family
  • Services Coordinator will provide support to family and awareness of community supports desired by family
  • Family wants to continue with Services Coordination

Child Outcomes:

  • John will have oral intake only of foods/liquids and eat from regular bottle
  • Austin to receive aged and disabled waiver services
  • Teresita will use many ways to let her family know what she wants
  • Shawny will lift and turn her head
  • Karla will use words or signs functionally (e.g.: requesting, making choices, and communicating wants/needs)
  • Tyson's gross motor skills to develop in sequential manner
  • Will meet developmental milestones
  • To monitor and facilitate age-appropriate cognitive/play skills and social skills
  • Ellie will demonstrate progression through developmental milestones in fine and gross motor skills (improved trunk strength, weight bearing on extended arms, and improved weight shifts) as observed by therapists
  • To continue normal developmental sequence
  • Improve fine and gross motor skills
  • Support play and speech development
  • Will check into getting vision and hearing checked
