Instructions for Page 7a: Goals and Outcomes

Goal/Outcome:
This page should be a record of one family/child outcome that would be possible in the next 6 months. Outcomes should be worded as observable events, not "improvements" or "increases" in specific behaviors.


For Example: Sonja will be able to sit with minimal support for play and feeding activities.

Child & Family Strengths:
This section should also list the strengths and resources available to the family for addressing the targeted outcome. This might include knowledge, skills and talents of individual family members, friends, relatives and associates as well as personal traits that make the promise of a successful program possible. Resources such as existing insurance, income, transportation, space, time, daily routines and easy access to information should also be considered strengths.

For Example:
Outcome:Janess will have a safe travel chair and other adaptive equipment for seating, standing and bathing at home and her child care setting.
Strengths: Mrs. Smith does some carpentry and can build some equipment herself. Mr. Smith works for Health and Human Services and can investigate possible financial assistance available for purchasing equipment.

What will be done by whom:
Finally, this section will document in 2-3 simple sentences that "tell the story" of what will be done to achieve each outcome in words that the family can understand. They will address who will do what, where, how often. They will also include mention of how the team (family and professionals) will measure accomplishments of each outcome.


For Example:
SLP will provide family with education/information about early vocal and non-vocal communication development through some written material and meetings weekly for the next month. SLP and teacher will coordinate home visits with the family (up to 6 hours total each month) to provide help and guidance for getting and then responding to clearer signals from Jonas. SLP's observation and parents' reports of at least six new signals in the next six months will be an indication of success.

Progress will be reviewed:
In this section, the team should record how often the program will be reviewed for progress and by whom. This can be done on a routine, regular basis, but it must be done at least once every 6 months. As progress is noted, new goals/outcomes can be targeted.

At the bottom of this page, there is space to document this progress. Additional space is provided on page 7b. Additional pages may be added.
