Instructions Page 11: Parent Signature Page

The parent/legal guardian for the child will sign this page. Their signature represents the parents' agreement to accept or reject some or all of the services listed in this IFSP. The family must be provided with a clear understanding of all that has been planned and agreed to before they are asked to sign this page. If the parent declines any service, they can use the space provided to explain their reasons for declining specific services (ie: services coordination, therapy) without risk of losing other services now or in the future.
