page will document who is responsible for which
services. Each agency service should be listed
and the agreed upon frequency, duration, and starting
and ending dates noted. Any services not provided
in natural environments should be justified on
this page.
The
words used to describe the services should reflect
the specific assistance/support to be provided.

noting who is responsible, the name of the discipline
or agency (or family) should be noted rather than
