Services
Coordinator Assigned
Once
the Services Coordination agency receives the referral,
a services coordinator will be assigned. The service
coordinator has seven calendar days to make the initial
contact with the family and meet with the family. This
time requirement may be waived at the request of the
family.
The services coordinator contacts the family for these purposes:
- to share information about the child and the System of Early Development Network services
- to start to develop rapport with the family
- to determine if the family is interested in proceeding with the process
- to begin to identify with families their daily routines, activities, and supports
- to gather information to address the family's concerns and questions about their child's health and development
- for families to begin identifying their resources, priorities and concerns
- to offer referrals for immediate needs
- to begin the process of verification of disability for special education, for children not already in the special education program
