Formal Exit
All
families should be informed from the very beginning
of the IFSP process that these services are considered
temporary. It is the intent
of the Services Coordinator and early interventionists
to do whatever is necessary to maximize the child's
and family's potential for independent daily life as
soon as possible. Secondly, the availability of all
services may be reduced as the child gets older (age
3-5 years) and different funding sources and eligibility
criteria are considered (Rule 51). Discussions
with family members should frequently address the eventual
transition from early intervention services and exit
from the IFSP process.
A
child/family may exit the IFSP process for a number
of reasons, including:
- MDT
decision that child is not eligible for Early Development Network ervices
- Child
shows considerable progress in response to intervention
services provided and no longer shows developmental
delays
- Family
decides not to accept or continue recommended/offered
services
- **Child
reaches his/her 3rd birthday (or the end of the school
year in which he/she turns 3)
When
it is determined that the family is not eligible for
early interventionservices, they are immediately
assisted in accessing services available in the community
they may be interested in pursuing. The services coordinator
is responsible for explaining to the family that they
are not eligible for ongoing services coordination.
The
family does have the right to appeal
this decision; however, the Services Coordinator
will not provide assistance or serve as an advocate
in the appeal process.
The
original referral source is notified of the
MDT and/or appeal decision if the child is not eligible
for or will not continue early intervention services.
The family must have written authorization, however
for this notice to be sent. This will be done in the
form of a letter outlining the outcome of the initial
and subsequent evaluations. The MDT report will also
be forwarded to the referring source if the family requests
this action in writing. The family shall receive a copy
of the letter that is sent to the referral source.
A
notice of action is sent to the family when Services
Coordination ends by the Services Coordinator, following
a meeting with the family.
