Understanding the Cornerstones

Family Centered Services
The phrase “family-centered services” reflects a way of coordinating and delivering assistance and support to families with children who have disabilities. The IFSP is designed to focus on the entire family and their needs, with families being the driving force behind the provision of services.

Family/Professional Relationships
The Individual Family Service Plan is intended to document the agreed upon goals and strategies parents and professionals have collaboratively worked out.

Effective Team Functioning
The Individual Family Service Plan is intended to be developed and implemented by a team of people, including the family and community service providers from various agencies.

Services Coordination
A Services Coordinator assists families of eligible infants or toddlers with disabilities within a community to identify and meet each child’s needs through coordination of formal and informal supports.
